BLACK MATRIX ACADEMY

Black Matrix Academy LLC does follow CDC guidelines to promote and prevent the spread of the COVID virus. Students that enroll, agree to adhere to current and future implementations set in place for the prevention of the virus. Training areas and training aids are cleaned and sanitized before and after every class. Students are not required to wear facial covering, but it is highly encouraged. Black Matrix Academy LLC will NOT permit access to individuals that show symptoms or refuse these measures.

We appreciate your decision to patronize our business for training. Precautionary measures are utilized to ensure the safety of students and clients while in our facility and visiting facilities. Safety is our priority.

The use of a firearm is a requirement in most of the services that Black Matrix Academy LLC offers and are expressly outlined in the individual and collective services. Enrollment and attendance to any and all services serves as an agreement that each attendee has reviewed our policies and acknowledge all liabilities in utilizing these services. Attendees, family members, agents, acting agents or any persons attached directly or indirectly to the student or heirs agree not to hold Black Matrix Academy LLC responsible or liable for any injuries or death.

Services require live fire qualification and/or training, students/clients are provided training and instruction to ensure that safety of fellow students and those sharing training space. If a member of Black Matrix Academy LLC staff or staff of an outside facilities utilized by Black Matrix Academy LLC believes or observes ANY student violating the safety rules provided verbally and or written, a disturbance or distraction, you will be disqualified from training and requested to depart classroom or gun range facility. Any and all violations of such as but not limited to those listed above will forfeit any future training with Black Matrix Academy LLC (at the discretion of the owner, instructor or trainers). Removal from training services for a violation of such will forfeit all monies and NO REFUND WILL BE PROVIDED.

Clients/Students agree to assume complete responsibility for their attendance to the facility in which they enroll. Failure to be at the training locations at the required or provided time, will result in a forfeit of payment or payments made partially or in full. NO REFUND WILL BE PROVIDED.

Appointment Cancellation 

When you book your appointment, you are holding a space on our calendar that is no longer available to potential students or clients. In order to be respectful of others, please contact Black Matrix Academy LLC as soon as you are aware that you will not be able to attend your appointment. This does not guarantee a refund but may allow the student/client to reschedule their appointment at the discretion of staff of the Black Matrix Academy. 

NO CALL/NO SHOW

Black Matrix Academy has a zero tolerance for NO CALL/NO SHOW. You will be allowed a 10-minute grace period to be present for training. If you are not present at the training facility within the grace period, it will be considered a NO CALL/NO SHOW and forfeits all monies paid to Black Matrix Academy LLC. You may reschedule for a future training session after a $40 rescheduling fee is paid. If any portion of training is missed without the approval of staff of Black Matrix Academy LLC refer to rescheduling fee. Please make all reasonable efforts to contact Black Matrix Academy LLC at (773) 726-3717 if you are running late within the grace period.

Rescheduling Fee

Rescheduling fee for any service will require a payment of $40. Rescheduling is at the set discretion of the staff of Black Matrix Academy LLC. All add on's such as but not limited to firearm rental and ammunition are included with the training if included in the billing invoice. If student requires additional ammunition, the cost will be at a fee of $20 for each additional time.

You agree that by submission of payment you have reviewed, understand and agree to the policies set by Black Matrix Academy LLC and are in agreeance with the set policies.
